Position Purpose :
Provides case management and supportive services for clients being treated in the Health Center and other units as needed.
Facilitate client engagement in treatment services and successfully transition clients through levels of care to support early recovery.
Provide continuity of program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards.
Participate as a team member in the delivery of addictions treatment services to clients and their families and to facilitate the issues of recovery into their daily living situations.
Qualifications / Basic Job Requirements :
- Bachelor’s degree in Psychology, Counseling, Social Work, Addiction Studies or other related field, or equivalent experience and training
- Demonstrated education, training, or experience in the behavioral health / human services.
- Excellent written and oral communication skills,
- Computer proficiency required,
- Ability to navigate and complete documentation in the electronic healthcare record,
- Ability to accurately complete required documentation within the prescribed time frames
- Ability to quickly establish rapport with both clients and families; and motivate engagement in support systems identified through case management.
- Ability to present and coordinate lectures for clients, families, staff, or community resources.
- Self-starter with ability to work independently and within a team environment.
- Valid driver’s license and ability to meet Agency insurance requirements,

Essential Responsibilities :
- Work hours prescribed by leadership and any additional hours deemed necessary for the responsibilities assigned.
- Manage client cases and develop actionable plans to address client needs, ensuring alignment with physician services and available resources.
- Finalize pending closures for physician services episodes and issue outgoing communication to clients along with supporting documentation.
- Identify and connect clients with relevant community health and wellness resources, including outside healthcare services, resources, and / or Rosecrance partners for physical, mental and material needs.
- Maintain accurate and up-to-date case files, including all third-party documentation work such as laboratory results, imaging, medication lists and psychiatric hospitalization discharge information.
- Assist with scheduling clients for psychiatric services as needed.
- Independently manage the schedule for medication-assisted treatment providers within the designated timeframe established by the provider.
- Refer clients to outside providers as appropriate with direction from provider, direct care, or nursing staff.
- Initiate and complete requested medical paperwork, including interim relief, FMLA, disability, and / or benefit documentation, to be forwarded to the provider.
- Assist with other duties as assigned which may include drug urinalysis, client and / or family education, and coordination with outside pharmacies.
- Serve as an effective team member, motivating and problem solving with staff to meet the complex needs of clients and their families.
- Adapt verbal and written communication to encompass the diverse language and communication styles of clients and their families, ensuring inclusivity and cultural sensitivity.
- Adhere to appropriate timelines when completing necessary documentation.
- Maintain consistent communication with all staff members.
- Utilize a variety of treatment concepts, evidence-based practices, and service modalities when providing care.
- Contribute actively as an integral member of the clinical team, fostering collaboration and inclusivity by participating in all team meetings, discussions, and activities aimed at enhancing client care.
- Exercise confidentiality in keeping with the professional Code of Conduct and within the framework of the law.
- Deliver exceptional customer service consistently to every customer.
- Serve as a role model and demonstrate positive guest relations in representing the agency.
Schedule : 8 hour shifts
Schedule : Tuesday-Saturday 8 : 30am-5pm
