Clinical Pharmacist- Solid Organ Transplant

The Solid Organ Transplant Clinical Pharmacist provides specialized patient-centered clinical services to the inpatient solid organ transplant population in addition to the outpatient transplant clinic / infusion center.

  • This position must : provide pharmaceutical care to patients to achieve optimal therapeutic outcomes; promote cost-effective drug therapy;
  • promote cooperative work environment by utilizing communication skills, interpersonal relationships and team building; assist with verification and dispensing of medication orders;
  • provide assistance in educating other health care professionals in regards to pharmacotherapy; participate in multidisciplinary and cross-service drug therapy management plans;

assist in developing and implementing treatment protocols in order to improve patient care. This person must possess the ability to work independently, demonstrate a positive attitude, have a strong commitment to teamwork and customer service as well as strive to consistently improve patient care by working as a cohesive transplant pharmacy team to achieve goals.

Provides clinical pharmaceutical care to patients focusing on abdominal, heart and lung

transplant

patient populations (inpatient and clinic).

  • Enhances the quality of delivery of care.
  • Demonstrates proficiency in Pharmacy distributive services.
  • Supports the educational and professional development of the Pharmacy department.
  • Engages in professional self-development.
  • Oversees the daily activities of the

transplant

transitions of care technician.

This position rotates every 6th weekend fulfilling clinical

transplant

pharmacy services.

In addition to the above job responsibilities, other duties may be assigned.

Position Compensation Range : $52.46 - $83.94 Hourly

MINIMUM REQUIREMENTS

Education : Bachelor's in Pharmacy or Pharm. D Degree required.

Experience : PGY-1 residency required or equivalent hospital experience. (3.years). PGY-2 residency is preferred for specialty areas.

Licensure : Commonwealth of Virginia State Board of Pharmacy. License required. American Heart Association (AHA) Health Care Provider.

BLS certification required

PHYSICAL DEMANDS

Job requires sitting for prolonged periods; Repetitive motion : typing. Proficient communicative, auditory and visual skills;

Attention to detail and ability to write legibly; Ability to lift / push / pull
