Social Worker - LMSW

What You Must Have :

• Master’s Degree in Social Work from a State accredited program.
• Current State license required in accordance with State regulations
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred
• Valid driver’s license, r eliable transportation, and provide ongoing valid and current auto liability insurance
• Experience in EMR a plus

What You Will Do :

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals
• Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements
• Attend Inter-Disciplinary Group (IDG) meeting, participate in the patient care planning process and collaborates with IDG to promote coordination of patient care
• Provide individual patient / family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues
• Facilitate community resources based on patient / family needs
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients / families as assigned
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures

Our mission is to provide culturally relevant and compassionate end of life care, grief support, and education to our patients, their families, and our community.

Our purpose is to ensure that appropriate end of life healthcare is available to ALL members of King County, particularly those traditionally underserved and underrepresented in our county.
