Position : Norman Case Worker
Reports To : Norman & YIT Supervisor
Status : Exempt
BASIC FUNCTION OF POSITION :
Responsible for assisting families with basic needs and resources. Helping families referred to become self-sufficient.
INFORMATION PRIVACY AND SECURITY LEVEL :
In accord with federal and state legislative mandates, individuals employed in this position are trained to, and function within a Level II-B security clearance for confidentiality of client protected health information, and other privileged information.
Appropriate sanctions will be applied against employees who fail to comply with the privacy policies and procedures of the Agency.
QUALIFICATIONS :
- 2 years of college at an accredited college or university (Associate's Degree or higher preferred) and two years' experience in the area of social service.
- Have a Motor Vehicle, a Valid Driver's license and the state required car insurance.
- Pass all State and Federal background checks.
PHYSICAL CHARACTERISTICS REQURIED :
- Ability to engage in daily, full-time attendance, and occasionally, in duties re quiring longer than eight (8) consecutive hours.
- Possess the ability to perform sitting, standing, and stooping in daily full-time employment.
- Ability to operate a motor vehicle during both day and night hours.
- Excellent proficiency in handwriting and computer use, and in the use of clear verbal and written communication skills.
- Have natural or corrected reasonably good vision and hearing, and the ability to use the telephone.
- The ability to move physically about the Agency's owned buildings and grounds, and to use stairways (especially in emergencies).
- Possess the ability to maintain calm efficiency and effectiveness in stressful and demanding situations.
PRINCIPAL DUTIES AND RESPONSIBILITIES :
- Coordinate all services for the current Norman families.
- Develop service plan with measurable goals.
- Complete Referral forms to necessary services
- Complete Social History within designated timeframe.
- Regular meetings with the Norman & YIT Supervisor and other staff to review cases
- Keep appropriate case notes and records
- Provide in home services when appropriate
- Complete closure reports
- Visit each client at least one time a week
- Coordinate on each family case and review service plan as needed.
- Facilitate networking and linkages with other appropriate services.
- Participate in Team Staffing as necessary.
- Follow-up on referrals for program.
- Perform other duties as assigned
