RN Case Manager
Kaiser is seeking a RN Case Manager for their Vandever Medical Office location. 30-40 hours per week 8a-5pm Monday- Friday.
Facility is willing to review candidates with messaging experience; Care Coordination for patients discharging from inpatient to acute rehab and all care related.
Approx 5 months assignment - set schedule most weeks. No weekends
Required : Demonstrated ability to utilize / apply the general and specialized principles, practices, techniques, and methods of utilization review / management, care coordination, transfer coordination, discharge planning or case management.
Working knowledge of regulatory requirements and accreditation standards (TJC, Medicare, Medi-Cal, etc.) -Demonstrated ability to utilize written and verbal communication.
Interpersonal, critical thinking, and problem-solving skills required.
Pay : $1785-$2380.00 gross per week. Based on a 30-40 hour work week. Paid weekly.
