The Health Specialist IV (Skilled Acquisition Plan (SAP) Writer is responsible for the comprehensive and holistic integration of the Individual Support Plan in all areas of each individual's life.
This includes integrating Individual Support Plans, Behavior Support Plans and PNMP plans (when applicable) in relevant environments that reflect the normal rhythm of each person's life.
This position involves developing structured training curriculums, organizing recreation / leisure activities and coordinating community integration.
This position takes an active leadership role in assessing, developing, implementing and monitoring all active treatment activities in assigned areas and ensures that specialized and generic active treatment activities occur based on individual preferences / choices and IDT identified strengths / needs.
The SAP Writer provides quality leadership through hands-on mentoring, coaching and training to all staff to ensure implementation of the individual’s programming and compliance with applicable standards on active treatment.
Other responsibilities include developing data systems to identify active treatment needs / trends, providing consultation / support to the Individual Support Teams, participating in applicable Individual Support Plans and Unit meetings, coordinating active treatment during mealtimes, and leading / participating in special project teams and committees.
This position writes skill acquisition plans in Applied Behavioral Analysis format under the direction of a consulting BCBA and the Director of Behavior Health Services.
This position is required to work a flex schedule and / or overtime to meet changing needs of the department. Essential Job Functions :
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and / or overtime in accordance with agency leave policy and performs other duties as assigned.
1. Identifies, develops, and implements ABA skill acquisition training objectives.
2. Provides hands-on coaching, teaching and mentoring to all staff in assigned areas on integrating active treatment programs and effective positive interactions.
3. Organizes the community integration component to ensure every individual participates in non-routine community activities and community skill acquisition planning.
4. Participates in Individual Support Plans, Preps, home meetings and other IDT meetings when applicable.
5. Develops and implements data systems to identify active treatment needs and trends of the facility. Synthesizes data into reports for distribution to appropriate departments.
6. Plans, schedules, and provides services to the homes in the delivery of evening, weekend, holiday and mealtime active treatment.
7. Supervises assigned staff in accordance with facility procedures to ensure the effective delivery of needed services.
8. Participates in special project teams and committees.
9. Completes and / or evaluates assessments on individuals such as the Functional Skills Assessments and assessments in all other disciplines.
Other duties as assigned include but are not limited to actively participating and / or serving in a supporting role to meet the agency’s obligations for disaster response and / or recovery or Continuity of Operations (COOP) activation.
Such participation may require an alternate shift pattern assignment and / or location. Knowledge Skills Abilities :

- Knowledge of and adherence to all policies and procedures related to maintaining confidentiality.
- Ability to perform CPR, First Aid, and SUR.
- Ability to follow facility shelter-in-place and hurricane evacuation policies and procedures.
- Flexibility in work hours will be required for this position. The position will be required to work overtime and / or extended hours and serve on-call after hours.
