Health Equity Strategy Lead at Humana in Boise, Idaho, United States Job Description Become a part of our caring community and help us put health first The Health Equity Strategy Lead develops the organization's strategic direction and coordination of health care equity to reduce or eliminate health disparities.
The Health Equity Strategy Lead works on problems of diverse scope and complexity ranging from moderate to substantial. The Health Equity Strategy Lead identifies policies and practices adversely affecting groups of people with social or economic obstacles to health.
Collaborates with leadership, physicians, departments and business units to promote the highest standard of health for all people.
Advises executives to develop functional strategies (often segment specific) on matters of significance. Exercises independent judgment and decision making on complex issues regarding job duties and related tasks, and works under minimal supervision, uses independent judgment requiring analysis of variable factors and determining the best course of action.
Reporting directly to the Director of Market & Community Integration, the Health Equity Strategy Lead will help execute market integration efforts by providing support to market operations teams to integrate health equity and social impact strategic priorities and initiatives in the market.
Additionally, the HESL will support collaboration and prioritization of national partnerships and programs to improve member experience, health outcomes and Humana's overall brand presence.
The Health Equity Strategy Lead (HESL) will be responsible for creating alignment with each Division's priorities through creating tools, resources and playbooks to build subject matter experts within operations teams.
Role Responsibilities : + Support HESI's Director of Market & Community Integration by developing tools and resources to effectively manage key community & market integration goals and objectives.
- Facilitate expansion of use for Humana Community Navigator (HCN) within market teams for Medicare, Medicaid and CenterWell.
- Drive tactical integration of HCN into clinician workflows and provider trainings. + Monitor and track performance on HEDIS SNS-E using HCN as a loop closure tool.
- Facilitate integration of Humana Community Navigator into internal platforms and track HCN usage / referral reporting.
