Position :

NursePractitioner Primary Care Provider

Responsibilities :

• Conduct inperson comprehensive assessments includinginitial semiannual and unscheduled assessments and providecontinuous medical coverage to participants.
• Collaborate with physicians and IDT to develop andimplement care plans for each participant.
• Participate in discharge planning from acute or longtermsettings in collaboration with physicians and IDT.
• Maintain participant care in nursing homes with regularvisits as needed.
• Work with WelbeHealthphysicians communitybased PACE physicians and IDT to ensureeffective care coordination.
• Review outcomesand utilization data including hospitalizations and specialist useto establish best practices and policy improvements under theMedical Directors guidance.
• Participate inrotating night and weekend call.

Requirements :

Required Qualifications :

• Graduation from an Accredited School ofNursing.
• Certificate as a Nurse Practitionerwith Prescriptive Authority.
• RN StateLicense.
• Minimum of two years of experienceworking with the senior population.
• Currentknowledge of chronic care / geriatric issues and bestpractices.
