Talbott Recovery is currently looking for a fulltime Continuing Care Coordinator / Discharge Planner for our Main Campus on Hunter Road, Atlanta.

Qualifications
Position Summary
Responsible for the coordination of administrative and discharge planning for all Talbott Recovery patients
Essential Duties and Responsibilities
- Establishes and maintains referral relationships.
- Contacting referral sources upon admission and confirms patient's needs / expectations regarding updates / information.
- Coordinates all communication and information exchange with
- Develops a comprehensive and patient-focused continuing care plan for assigned patients.
- Timely and accurate input of contact information into established system.
- Ability to access, assess and report on requested and / or needed information.
Education and Experience
- Experienced in alcohol / substance abuse / 12-Step program.
- 1 to 2 years of such experience preferred
- Must have a master's degree in related field
- LCSW, LMSW, LPC, LAPC or Addiction Certification is preferred
