from anywhere within the U.S. as you take on some tough challenges.Primary Responsibilities : Completes post discharge assessments evaluating members for clinical and social needs timely and effectivelyFollows model of care requirements as outlined in policies, procedures, and job aidsEducates members on disease process and medications to include completion of the Medication Reconciliation - Post DischargeEvaluates members for ongoing needs and completes referrals as appropriateManages, communicates and updates plan of care with membersCollaborates with other programs and departments as necessaryCollaborates with Primary Care Physicians regarding medication concerns, questions, or other member needsAbility to work independently and follow program processesYou’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.

Required Qualifications : Current, unrestricted RN compact license3+ years of experience in a hospital, acute care, or direct care settingBasic level of experience with Microsoft Office (Word, Excel, Outlook)Designated workspace and access to install high-speed internet via hardwire in homePreferred Qualifications : Bachelor's DegreeCertified Case Manager (CCM)Experience working in an outbound call center environmentExperience or exposure in discharge planning, utilization review, concurrent review, risk management or short-term case managementExperience in a telephonic roleProven solid organizational skills and multitasking abilitiesProven ability to work in a fast-paced environmentBilingual, Spanish*All employees working remotely will be required to adhere to UnitedHealth Group’s Telecommuter PolicyCalifornia, Colorado, Connecticut, Hawaii, Nevada, New Jersey, New York, Rhode Island, Washington or Washington, D.

C. Residents Only : The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.
