Advanced healthcare integration is a mobile provider service with a goal to decrease hospital readmissions. We are looking to hire a highly dedicated, detail oriented people with drive to help our company grow.
This position will receive incoming referrals and create provider schedule for home visits. Patients must be seen in a 24-48 hour time frame.
You’ll also be responsible for chart preparation and communication with providers. Having a medical assistant background is highly preferred.
