The YMCA of South Florida is now hiring a Success Coach to join our Youth Development Team in Broward County. The Success Coach has the overall responsibility for the operation of a STEP program site serving an average of 25 High School students with special needs between the ages of 16 and 21 .
The position is responsible for providing transition case management services to teens / families in accordance with the requirements of the contract and principles of the YMCA.
The position is also responsible for the development of transitional plans and connections to community resources which lead to student's employment or secondary education.
Transition planning includes is but not limited to referrals / informational assistance with education, vocational programs and / or employment resources.

Essential Functions :
Supervises the Transition Supervisor at a designated site and carries out all the supervisory responsibilities in accordance with the YMCA of South Florida policies and all applicable laws.
Supervisory responsibilities include planning, implementing assigned curriculum, assigning and directing work, evaluating and disciplining employees and working directly with the youth served.
- Responsible for all case management associated to all students
- Appropriate Documentation.
- Oversees and participates in the implementation of prevention / education curriculum and presentations.
- Ensures developmentally appropriate transition, employment education, and cultural enrichment activities are offered to students.
- Coordinates on-site program activities, summer employment and volunteer opportunities.
- Provides training to staff and supervises Transition Specialist.
- Completes all participant registrations, assessments, and follow ups and recording of student progress will be completed weekly.
- Responsible for meeting the needs of students to ensure their success and having regular communication with teachers, schools, parents and other stake holders.
- Assist Program Director in coordinating field trips, guest speakers, job fairs, community outreach partnerships.
- Adhere to the Child Abuse & Neglect Reporting requirements which mandate by law all child care personnel to report suspicions of child abuse, neglect, or The YMCA requires employees to follow the internal reporting procedures with the assistance of the Program Director.
- Ensure that the proper staff to child ratios is maintained at all times and reports discrepancies to the Program Director as needed
- Assist the Program Director in any other appropriate ways as directed.
Job Requirements :
- Bachelor's degree in Social Work, Psychology, or related field, or equivalent years of direct experience in Case Master's degree preferred.
- At least one (1) year experience of case management.
- At least one (1) year experience working with individuals with physical or developmental
- At least one (1) year working in transition / job employment education activities or one (1) year working in job placement / job coaching.
- Must have the ability to communicate effectively with a diverse group of employees, volunteers and program
- Must possess the ability to complete caseload documentation
- Strong communication skills in both written and verbal language also
- Must possess exceptional conflict resolution skills.
- One year of experience in supervision
- Ability to relate to people and work with a diverse population; understand and practice YMCA Character Development values of Caring, Honesty, Respect and Responsibility.
Certificates and Licenses :
- Current CPR and First Aid certificates by time of employment or within ninety 90) days from the date of hire,
- It is the responsibility of the Case Manager to maintain all necessary certifications set forth by either CSC funding agency, State of Florida or the YMCA of South Florida.
