Developing referrals for concierge nursing services with key referral sources including : hospital discharge planners, social workers, case managers, and physicians, as well as community Assisted Living, and Rehab facilities.

Requirements

  • Established relationships with potential referral sources for concierge health care services.
  • Prefer experience in home care and related services
  • professional demeanor and presentation
  • Competent in computer-based skills including internet, email, Microsoft Office, and CRM software

Clinical background a plus, not required
