RESPONSIBILITIES :
- Community Liaison / outreach workers will complete surveys to Pact Rad community and collect data that will assist in identifying disparities that affect health and wellness such as diabetes, high blood pressure, heart disease, limited doctor’s visits etc.
- The Community Liaison will gather Information via the surveys and use the information to develop service plans based on the community needs.
- The Community Liaison will assist the program coordinator to present Workshops that address the most pressing needs based on the survey results.
- The Community Liaison will interact with the participants on a weekly / biweekly basis and keep participants abreast of upcoming workshops.
- Assist Program coordinator with onsite preparations for workshops
- All other duties as assigned by coordinator.
QUALIFICATIONS :
- High School diploma or GED. Some College Preferred
- Experience in the non-profit sector
- Familiarity with our benefits access program
PHYSICAL REQUIREMENTS
- Full vaccination against COVID-19 is required
- Ability to use a computer for prolonged periods
- Ability to occasionally lift and / or move up to 10 pounds
- Ability to stand, walk, or sit for long periods of time
- Ability to bend and retrieve objects and / or documents
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and / or field work.
