An opportunity has arisen with a highly regarded healthcare organization for a California Health System for the role of Director of Care Management in their largest and most well-respected hospital.
The role encompasses clinical and administrative oversight, including care coordination, case management, and various healthcare programs in both inpatient and outpatient settings.
The Director will be accountable for ensuring patient care that is high-quality, safe, efficient, and patient-centered. This leader will collaborate with multiple departments, physicians, and executive leadership to drive performance and strategic initiatives aligned with organizational goals.
The role involves direct involvement with utilization management and throughput processes, ensuring optimal performance against key metrics such as length of stay, readmissions, and financial outcomes.
The ideal candidate will have extensive experience in clinical operations, lean management, and performance improvement. They will be responsible for managing relationships with community partners, driving operational efficiency, and maintaining compliance with regulatory requirements.
In addition, this role includes overseeing budget performance and staffing resources to meet the department’s goals and objectives.
Key Responsibilities :
- Lead care transitions and clinical services for designated hospitals and departments.
- Oversee the continuous improvement of care quality, safety, and patient experience.
- Ensure effective utilization of resources to meet fiscal and operational targets.
- Develop collaborative relationships with medical staff, leadership, and community partners.
- Drive performance on key metrics, including patient throughput, length of stay, and financial outcomes.
- Ensure compliance with accreditation and regulatory requirements.
- Promote staff development and retention, ensuring the workforce is well-equipped to meet patient care needs.
- Lead strategic initiatives to optimize care delivery and patient outcomes.
