The IHH Care Coordinator’s primary responsibility is to address the needs of adults with serious mental illness using a person center approach to provide support and increase a client’s ability to function and live in the community.
This position will provide a whole-person model of care coordination across medical, behavioral, and social service community services.
Salary Range : $37,250 - $51,400

For full-time employees after their first year of employment, one hour of vacation time is earned for every 13 hours paid, with a maximum of 160 hours earned per year.

The IHH Care Coordinator’s job Duties :
- Participates in all Person Center Service Plan meetings, Comprehensive Assessments, care activities, and case discussions with service providers and client on a regular basis.
- Routinely communicates and coordinates care and services with the whole IHH team, Peer Support, Nurse Care Manager and other Community Mental Health Center services.
- Acts as a resource for the client and other providers for care issues, referral for services, and needed services and interventions.
- Coordinates with outside providers to ensure IHH clients receive support to meet their basic needs, receive appropriate mental health care, crisis intervention, housing support, family support, and advocacy services.
Requirements :
IHH Care Coordinator Requirements :
- Bachelor of Arts / Science in a human service field.
- One year related experience.
- Knowledge of psychology, social work, and or education fields and knowledge of community resources pertaining to the needs of a person with a serious mental illness.
- Ability to use technology and information systems effectively and as directed.
- Excellent communication and interpersonal skills.
- Exhibit a highly professional presentation and manner.
- Ability to work in a fast-paced environment and maintain composure under stress.
- Prior experience working within a community mental health center preferred.
- Ability to work cooperatively with staff and other services providers, and interface effectively, sensitively with clients / families.
- Have a valid driver’s license, meet qualifications to operate agency vehicles, and have acceptable auto insurance coverage.
- Have the ability to fulfill all of the regulatory requirements for employees of a Community Mental Health Center in the State of Iowa as required by state and federal law.
This includes successful completion of the criminal background investigation, as well as the office of the inspector general exclusionary screening.
