Key Responsibilities
Physician Assistants provide primary and / or consultative professional care in a variety of care settings. The Physician Assistant will function in collaboration with physicians according to a Collaboration Arrangement. The Physician Assistant practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
In this role you will be responsible for :
Secure patient signatures on necessary documents and properly document in EMR when necessary at a visit. Review HIPAA Notice of Privacy, consents for treatment, release of records and patient responsibilities forms with patients / caretakers.Drives between locations, navigating to each location using GPS device or other tools if required.Assess health status by obtaining relevant health and medical history. Performs a physical exam based upon the age and patient history as per the Collaborative Agreement with a collaborating physician in the patient's home environment. Identifies significant health and medical risk factors.Performs Annual Wellness Visits and other quality driven assessments to provide patient with preventative services and understanding of disease progression.Orders, interprets and evaluates laboratory tests and other diagnostic studies to assess patient's clinical problems and health care needs. Develops a diagnosis by analyzing and synthesizing collected data.Performs diagnostic or therapeutic interventions consistent with certification, law and state medical privileges. May prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board AgencyDevelops a treatment plan in collaboration with the patient and significant others. Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members to ensure quality patient care.Refers patients to physicians as appropriate for consultation or for specialized health resources and treatment. Ensures continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas as necessary.Provides patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits.Required Qualifications
Master's or doctoral degree as a Board-Certified Physician AssistantCurrent State License as a Physician AssistantSit, stand, bend, lift, and move intermittentlyMust be able to lift at least 35lbs and bear the weight of an average adult effectivelyCurrent and valid driver's licenseReliable transportation and valid automobile insurance coverageDemonstrates skill in physician assistant practice, ability to cope with family emotional stress and tolerance of individual lifestylesAbility to handle multiple priorities, documentation requirements and deadlinesDesired Qualifications
2 years clinical experience in a post-acute or as a Physician Assistant preferredExperience with geriatric patients preferred.Familiarity with Aprima EMRLocation
