RESPONSIBILITIESI. JOB SUMMARY / RESPONSIBILITIES :
Implements, maintains and provides clinical pharmacist services including : clinical, educational, informational and distributional functions for newborn, infant, pediatric, adolescent, adult and geriatric patients of The Queen’s Medical Center (QMC).
II. TYPICAL PHYSICAL DEMANDS :
- Essential : Standing, sitting, stooping, bending, walking, finger dexterity, seeing, hearing, speaking.
- Frequent : pushing / pulling usual weight of up to 40 pounds of force, lifting or carrying up to 30 pounds, reaching above, at and below shoulder level, frequent gripping of an object.
- Continuous : repetitive arm / hand motions.
- Occasional : kneeling, crawling, climbing stairs, walking on uneven ground, squatting, static gripping of an object for prolonged periods, pushing / pulling up to maximum 50 pounds.
- Operate pagers, computers, printers, typewriters, telephones, fax machines, dispensers, laminar flow hoods, TPN compounding machines and other pharmaceutical and office equipment.
III. TYPICAL WORKING CONDITIONS :
- Not substantially subject to adverse environmental conditions.
- Subjected to noise / vibration from air conditioner, printers, time stamp machines, pneumatic tubes, telephone, etc.
- Exposure to hazardous drugs, antibiotics, chemicals, dust, odors and cleaning / disinfectant fumes.
- Work environment is fast paced, congested and may be stressful due to workload.
- Interacts with patients, nurses, physicians and the general public.
- Works on multiple tasks at the same time.
- May be exposed to communicable diseases.IV. MINIMUM QUALIFICATIONS : A. EDUCATION / CERTIFICATION AND LICENSURE :
- Current Hawaii State license as a Pharmacist.
- Doctor of Pharmacy degree preferred.
- Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists preferred.B. EXPERIENCE :
- Clinical hospital pharmacist experience preferred. Preferably with experience in rounding with a medical team, monitoring of patient drug therapy, pharmacokinetic dosing and patient counseling and education.
- Experience in unit dose and IV services preferred.
