Responsibilities
Essential Functions
Accurately collects and validates the home medication list through patient interview and primary source verification with dispensing pharmacy, primary care physician or specialists
Monitors all patient medication orders for correctness of therapy, while considering allergies, dose, concurrent medications, interactions and diagnosis.
- Provides patient education throughout the hospitalization
- Assess patient health care literacy and modifies teaching methods appropriate to educational level
- Identifies barriers to medication adherence and develops action plans to mitigate patient risk and foster adherence.
- Develops comprehensive medication management plans and communicates this plan to hospitalists and primary care physicians
- Contacts prescriber about questionable orders and provides information on the correct therapy, noting formulary alternatives when indicated
- Educates clinicians regarding medications.
- Prevent, detect, monitor, document and report adverse drug reactions and medication errors
- Facilitates compliance with core measures, prospective formulary management and appropriate documentation in the medical record
- Participates in ICU rounds and interdisciplinary rounds and other committees as needed.

Coordinates care with other healthcare providers.
When appropriate, delegates to other healthcare providers.
Assesses and reassesses the patient’s condition, status and progress.
Prioritizes, reprioritizes, and modifies the plan of care based upon patient assessment and patient condition.
Documents the implementation of the plan of care and any adverse and / or unexpected events or outcomes.

Job Requirements
Minimum Education
Pharmacy and current MA licensure.
Required Skills
- Clinical background to help develop programs
- Good communication skills to relate with medical and nursing staff.
