Job Description
The social worker is a member of an interdisciplinary team responsible for meeting the psychosocial needs of the patient.
Responsibilities may include case management, advocacy, and planning services. The social worker may also facilitate the development of a discharge plan and may coordinate after care continuity.
Hard Skills
- LCSW or LMSW
- Medical experience (nice to have)

This is a contract position with a duration of 3 Month(s).

This is a fully on-site position in New Britain, Connecticut.
