We are looking for a dedicated Patient Advocate like you to be a part of our team.

Job Summary and Qualifications

As a Patient Advocate / Quality Outcomes Coordinator, you will facilitate quality and change management processes throughout the facility.

Under the supervision of the Director of Quality, the Patient Advocate / Quality Outcome Coordinator manages and assumes responsibility for quality data management including, but not limited to : complaints, grievances, family / patient-centered meetings, patient rounding, and facilitation of other portions of the grievance process.

What You Will Do In This Role :

• Oversight of the complaint and grievance process including : CMS, regulatory requirements, family / patient-centered meeting organization and facilitation, and formal responses to grievances
• Completes daily inpatient rounding to support nursing staff and enhance patient satisfaction
• Supports nursing competency validations in patient experience, bedside shift report, hourly rounding, safety rounding, and AIDET
• Tracks, analyzes and reports process successes and barriers
• Provides data and information in manner that assists with detection of patterns, trends, and opportunities for improvement.
• Participates in rounding related to regulatory compliance as required.
• Collaborates with unit management to implement best practices
• Collaborates with the Director or Risk manager to identify areas of risk.
• Collaborates with the Director of Patient Experience to complete tracking processes.
• Participates in hospital or departmental performance improvement activities, and seeks opportunities to improve department and inter-department processes.
• Collaborates with the leadership team to assure policies and procedures are correct.
