Position Summary

As an Ambulatory RN Care Manager you’ll be responsible for exemplary cross-continuum care coordination, orchestrating care across the health care system and identifying opportunities to improve patient care, services and overall population health.

You’ll employ the nursing process in the delivery of patient-centered nursing care and work collaboratively with multiple disciplines and resources facilitating wellness, and transitions of care for patients in the ambulatory environment.

Responsibilities

  • You will ensure the patient and family experiences seamless and safe transitions of care and engage with them as an integral and essential component of the care delivery process
  • You will advocate for patients and families, ensuring sensitivity to cultural and ethnic practices and beliefs
  • You will provide patient-centered education of disease processes and wellness opportunities
  • You will assist patients and families toward self-management of care including goal setting for management of chronic disease, improving medication adherence, creating a plan for behavioral change and coordinating community resources via nurse visit, telephone management, or other contacts, as needed
  • You will collaborate with leadership and the care team to identify opportunities, and develop a plan of action to promote appropriate utilization of resources across the continuum
  • You will serve as resource for staff, colleagues, physicians and advanced practice providers to positively impact CMS core measures and National Patient Safety Goals.

Requirements

  • Bachelor's degree required; preferably in nursing but other disciplines may be accepted
  • Current Registered Nurse licenses in Washington State or Multistate license endorsement (MLS)
  • Master’s degree in nursing preferred
  • Certification as Certified Case Manager (CCMC), Accredited Case Manager (ACMA), Care Coordination and Transition Management (AAACN), Ambulatory Care Nursing (AAACN), or similar required or obtained within 3 years of hire (Employees hired prior to 8 / 1 / 2023 will have this requirement waived)
  • Three (3) years of recent clinical hospital / ambulatory experience
  • Knowledge of care continuum (inpatient, ambulatory, post-acute, etc.) preferred

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off.

As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position.

The pay scale is $107,432.00 - $179,067.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to : skill set, level of experience, and certification(s) and / or education.

If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
