Clinical Program Director
Licensed Social Workers are eligible for a $5000 signing bonus!
As a Clinical Program Director for the Community Transition Time Program (CTTP), you will oversee a new model of care for youth to keep them out of unnecessary hospital settings.
You will manage a multi-disciplinary team that provides services and support to adolescent youth and their families in the community and short-term residential settings.
Responsibilities will include the management of the program, staff matters, client referrals, client treatment, financial viability, marketing of the program, interfacing with referral sources including local hospitals and county mental health providers, program manual, and program systems.
You will report to the Senior Director of Residential Services and will work onsite.
Responsibilities :
- Lead on recruiting, hiring and onboarding team members.
- Provide overall supervision to the team.
- Monitor caseload size and coordinates program intakes and discharges.
- Provide training and supervision for the Staff.
- Oversee a short-term eight bed residential unit that provides 24 / 7 care.
- Make rotating on-call schedule for after hours and weekend coverage by team members.
- Become subject matter expert in this new program model and participate in collaborative meetings with the state and other agencies.
- Educate the community about Community Transition Time and Intervention program, outcomes, theoretical background.
- Develop and maintain collaborative relationships with other agencies, schools, colleges, institutions, hospitals, primary care providers, housing authorities, and law enforcement agencies to help meet the needs of this population through timely referrals, connections and warm handoffs.
- Ensure staff are adhering to the fidelity of the program.
- Participate in all required trainings as directed.
Schedule / Shift :
- Monday - Friday, day hours
- Telephonic on call hours
Requirements and Education :
- A Master’s degree and license in a helping profession required (Social Work, Mental Health Counseling, Marriage and Family Therapy, Creative Arts Therapy, etc.)
- At least two years of progressively responsible related experience.
- Valid NYS driver's license is required
