Our team of highly trained providers are equipped to address various concerns including, but not limited to, chronic migraines, neck / low back pain, neuropathy as well as strokes, seizures, and memory loss.

We additionally offer diagnostic testing, procedures, and several resources to aid in our patients health care journey.

We are seeking a dedicated and compassionate patient coordinator to join our team. This role is a crucial part of our mission to provide exceptional support and assistance to individuals with chronic health conditions.

As a Patient Coordinator, you will play a pivotal role in the healthcare team by overseeing and facilitating the care of patients with chronic conditions.

You will be responsible for coordinating the comprehensive care management program and working closely with patients, healthcare providers, and our team of dedicated professionals.

Job Responsibilities :

Reach out to patients with chronic conditions to educate them about our care management services and establish a positive and supportive relationship.

Conduct comprehensive assessments of patients' health status, create individualized care plans, and set realistic health goals in collaboration with healthcare providers.

Coordinate and manage all aspects of patient care, ensuring that services are timely, effective, and aligned with their care plans.

Monitor medication adherence, educate patients about their medications, and collaborate with pharmacists and physicians to optimize medication regimens.

Provide ongoing education to patients and their families about their chronic conditions, self-management strategies, and lifestyle modifications.

Conduct regular follow-up calls and appointments to track patient progress, address concerns, and adjust care plans as needed.

Maintain accurate and up-to-date electronic health records, documenting all interactions, assessments, and care plans.

Collaborate with healthcare providers, specialists, and other members of the healthcare team to ensure continuity of care and optimal patient outcomes.

Participate in quality assurance initiatives and evaluate the effectiveness of care management programs to continuously improve patient care.

Ensure that all care management activities adhere to relevant healthcare regulations and standards.

Qualifications :

  • Medical Assistant 3-5 years experience
  • Previous experience in care coordination, chronic disease management, or a related healthcare role.
  • Strong interpersonal and communication skills.
  • Proficiency in using electronic health record systems.
  • Knowledge of chronic health conditions and their management.
  • Compassion and empathy for patients with chronic illnesses.
  • Organizational and time management skills.
  • Ability to work independently and as part of a collaborative healthcare team.
