Overview
The Care Navigation Nurse Practitioner is an active member of Capital Caring’s Medical Staff and provides phone-based care management to our patients and families throughout our continuum of care.
This includes, in part, home and facility-based hospice, home and facility-based palliative care, inpatient hospice, hospital-based palliative care consultation and outpatient palliative care clinics and Primary Care at Home.
He / she provides clinical support to the Care Navigation (CN), to Triage Staff and Interdisciplinary Teams (IDTs) during evening and weekend hours to provide support to patients and families discharged from the hospital.
The CN Nurse Practitioner will develop methods to improve clinical outcomes which ensure the continued quality of care, as well as guarantee timely communication of evening and weekend phone-based care management activity.

The Care Navigation Nurse Practitioner (CNNP) advocates for patients by helping translate complex medical information into language they can understand which then makes information that can be overwhelming more manageable.

Experience Requirements
Must have 5 years of recent clinical experience including sufficient experience in hospice, palliative care, and / or acute inpatient care.
Education Requirements
Must have a Masters degree in Nursing and completed an accredited Nurse Practitioner program.
Required Certificates and / or Licenses
Must have medical licensure from the governing body for medical practice for the District of Columbia, State of Maryland, and the Commonwealth of Virginia.
These licenses must be obtained or apllied for within 30-days from hire date. Must maintain medical licensure, perscriptive authority, and DEA licensure for all Capital Caring Health jurisdictions.
The Nurse Practitioner national certification is required. Also must currently have, or obtain advanced certification in hospice and palliative nursing within 2 years of the date of hire (HPNA)
