This is a full-time, FNP home clinic role supporting the Suwanee clinic at 5370 Laurel Springs Parkway. *Must be willing to work every other weekend *CVS Health is seeking a Nurse Practitioner (NP) to join our Primary Care team.

In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients.

The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.

This role will report to the practice manager.PRIMARY DUTIES AND RESPONSIBILITIES : Responsibilities include but are not limited toPatient-Centered Quality and SafetyUses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patientsPerforms history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.

Provides patient counseling; inclusive of pregnancy prevention, STI Prevention / safer sex practices, contraceptive care counseling and medication managementCounsels and educates patients and families on health maintenance and respond to patient care inquiriesEvaluates and treats patients' medical needs with safe and high-quality treatmentDocuments all patient care within an electronic health record in accordance with current policies and proceduresWorks as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patientsProvides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of diseaseRefers to appropriate specialists and patient care components as appropriateMaintain patient confidentiality in accordance with PHI and HIPAA standardsCommunication and Interdisciplinary CollaborationCollaborates with clinical care team to ensure the delivery of high quality, patient centric, longitudinal careParticipates in and in some cases leads clinical care team huddlesDemonstrates core values of CVS Health in all communications and interactionsCommunicates effectively with members of the clinical care team, other health professionals and health-related agencies.

On call rotation shared with providers in market for after hours patient clinical needs or shift coverageWorks effectively with clinical care team membersPromotes mutual respect and trust working within a team model.

Maintains self-awareness and professionalism of individual actions and how they impact the practice, and healthcare industryContinuing education, including what is required to maintain employment at CVS Health / Primary Care, is the responsibility of the providerEDUCATION : Completion of master’s degree level Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role required.

Required Experience : A minimum of 1 year experience as a Nurse Practitioner is required. Experience in Family medicine or primary care practice preferred.

Internal applicants with less than 1 year experience are eligible with successful completion of the CVS Minute Clinic Primary Care Training ProgramActive Advance Practice Registered Nurse, Nurse Practitioner license in good standingCurrent National Board Certification in Family PracticeADDITIONAL REQUIRED SKILLS AND ABILITIES : Working knowledge and understanding of quality measurement in the management of chronic disease conditions.

85 - $90.13This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.

The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
