HCT seeks an experienced nursing professional to serve as the Director of Case Management for a 350+ bed hospital in California.
Responsibilities include, but are not limited to :
- Oversight of the daily operations of the Care Management and Social Services departments.
- Developing and ensuring adherence to departmental best practices and policies as well as providing education.
- Establishing outstanding rapport and coordinating with physicians, clinicians, health plans, and local providers such as skilled nursing facilities, long-term care facilities, home health agencies, and ambulance companies.
Requirements :
- Education : BSN degree required; Master's degree in related field preferred; RN required.
- Experience : Minimum 5 years progressive experience in a Management or Administrative capacity in Hospital Case Management leadership.
Knowledge of DRGs, Medicare Regulations, Medical Necessity Criteria, and associated knowledge specific to Case Management.
Experience with State & Federal Regulations, including Title 22 and other regulatory requirements.
- Licensure : Current CA RN license required.
- Certifications : BLS required. Specialty certification(s) in the areas of Case Management and / or Administration is preferred.
- Computer Skills : Meditech.
