Heartland Hospice, part of the Gentiva family, is seeking a Hospice Social Worker MSW (Per Diem) to join our team. The focus of this role is to provide psychosocial support to patients and their families through assessment, interventions, and services in accordance with the established plan of care.
A Master's Degree in Social Work and state licensure are required, along with at least one year of experience in a healthcare or hospice setting.
