Description : POSITION SUMMARY

POSITION SUMMARY

The Referral Coordinator is responsible for managing the medical referral process from the initiation of the order to completion.

This position responds to patient and specialist requests and concerns, along with and establishing and maintaining positive relationships.

When needed, this position provides back-office clinical support.

Scope of Tasks and Responsibilities

  • Utilize the RHC referral protocol to process referrals, ensuring patients are sent to the appropriate specialist for diagnosis or treatment within a timely manner.
  • Process all STAT orders the day they are received. Then prioritizing work from urgent referrals to regular referrals.
  • Provide support and counseling as appropriate to patients, ensuring a complete transfer of information.
  • Facilitate the scheduling of STAT and Urgent appointments and inform referring provider with referral appointment status
  • Gather clinical information from chart, including CPT and drug codes, which may be used for the course of treatment
  • Create and maintains working knowledge of all managed care entities, ensuring that referrals are sent to appropriate locations
  • Maintain RHC database of all referral resources, completing updates on a continual basis
  • Monitor and track referrals sent out, completing follow up calls to all entities to promote appointment compliance
  • Utilize medical knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient / family as evidenced by documentation and observation of positive patient care outcomes
  • When / if working the back-office, perform delegated independent medical functions using established procedures, policies, guidelines, and standards as observed by the registered nurse
  • Maintain accurate and timely records and ensures documentation is updated regularly when following the referral status
  • Attend regularly scheduled staff meetings
  • Complete all assigned training
  • Collaborate with the Clinical Operations Director to review special cases and improve practice processes
  • Treat patients, visitors and co-workers with respect and dignity
  • Exemplify the RHC mission through a personal example of excellent service to patients, visitors and coworker
  • Attend required annual in-service programs
  • Cross train with other administrative positions as required, ensuring continuity of care for RHC patients
  • Certified MA’s may fill in for clinical needs during critical staffing periods
  • Other duties as assigned

Requirements :

Required Education, Certification, Experience and Skill :

1. Post high school certificate / education in Medical Assistant or valid registration as a Licensed Practical Nurse.

2. Two years’ experience in medical / clinical setting preferred

3. Excellent communication skills both verbal and written; customer service focused

4. Ability to take initiative, use independent judgment and troubleshoot when issues or difficult situations arise
