JobDescription
Whatyou will be doing : Nurse Practitioner / Primary CareProvider
- Perform inpersoncomprehensive assessments including initial semiannual andunscheduled assessments and continuous medical coverage toparticipants
- Collaborate with physicians andIDT to develop care plans for each participant
- Collaborate with physicians and IDT in discharge planningfrom acute or longterm settings
- Help maintaincare of participants in the nursing home making regular visits asdictated by participant need
- Workcollaboratively with physicians communitybased PACE physicians andIDT to oversee and ensure effective care coordination
- At the direction of the Medical Director review outcomesand utilization data including hospitalizations and use ofspecialists to establish best practices and policyimprovements
- Take part in rotating night andweekend call
Experience you willneed : Nurse Practitioner / Primary Care Provider
- Graduation from an Accredited School ofNursing
- Certificate as a Nurse Practitionerwith Prescriptive Authority
- NP StateLicense
- Minimum of two years of experienceworking with the senior population
- Currentknowledge of chronic care / geriatric issues and bestpractices
