Job Summary :
The Nurse Navigator provide patients with specialized care by guiding them through the complexities of the healthcare system and outline patients' diagnoses and their treatment options in accessible terminology.
Duties / Responsibilities :
- Supports patients throughout the healthcare process, from detection to treatment and beyond.
- Educates patients about their diagnoses, treatments, and the side effects of these.
- Provides resources for patients seeking reliable information.
- Helps patients to overcome barriers such as financing or transport.
- Refers patients to Social Workers or Counselors, as needed.
- Manages timely appointment scheduling for tests and procedures.
- Answers questions or coordinates with doctors to address questions and concerns.
- Provides emotional support and empowerment to patients and their loved ones.
- Develops standards of care, practice guidelines, and diagnosis-based protocols, and ensures patients receive quality care though use of these methods.
- Communicates effectively and professionally using all modalities while maintaining patient confidentiality in accordance with HIPAA regulations.
- Documents appropriate information in the patient’s permanent medical record.
- Attends, participates, and / or assists in meetings, trainings, community outreach activities, continuing education opportunities, and other activities as required.
- Performs other related duties as assigned.
Qualifications
Required Skills / Knowledge / Abilities :
- Extensive experience working with patients in critical conditions.
- Sound knowledge of the healthcare system and where to find reliable information.
- Excellent verbal and written communication skills.
- Openness towards different cultures, beliefs, and traditions.
- Sensitivity, discretion, and respect.
Education and Experience :
Required :
- Associate’s degree in nursing.
- Registered Nurse (RN) State and / or compact state licensure.
- Basic Life Support (BLS) certification.
- 2+ years of nursing experience.
- 1+ years of experience in a clinical setting.
Preferred :
- Bachelor’s degree.
- 3+ years of experience and / or certification in a designated specialty.
Other Requirements :
- New Hires are required to pass pre-employment background check and drug testing (effective 11 / 1 / 2022).
- Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire.
