SUMMARY :

Community based health promotion and disease prevention specialist for the Lower Sioux Indian Community Members. Overall responsibility for planning, directing, managing all functions, operations, and programs for the Lower Sioux Community Health Department.

Nutrition Professional responsible for Menu planning for the Elder Nutrition Program, diet counseling for diabetes and other nutritional diseases.

EDUCATION AND EXPERIENCE :

  • Bachelor’s degree in health care required, master’s degree preferred.
  • Possesses nutrition knowledge to care for patients across the life cycle.
  • Excellent verbal and written communication skills such as communicating with a broad range of individuals.
  • Experience in grant writing preferred.
  • Knowledge or experience working in a tribal community.
  • Previous experience in a Public Health Setting
  • Supervisory and Management Experience.
  • Valid Minnesota Driver’s License.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

  • Researches opportunities to expand the Public Offerings for the Lower Sioux Community.
  • Coordinate and carry out the Special Diabetes Program for Indians grant funded program.
  • Serve as the Title VI Director for the Elder Nutrition Program.
  • Direct and monitor budgets for all Lower Sioux federal, state, and non-profit community health grant funded programs.
  • Direct and provide oversight to strategic planning and personnel management.
  • Supervises all Community Health employees.
  • Build and maintain collaborations with local, regional, state, and national partners to achieve department strategic goals.
  • Pursue available funding streams to expand programs and provide sustainability in programming.
  • Write grant proposals for state, federal and private funding mechanisms.
  • Completion of all reports as required by IHS and the Lower Sioux Community.
  • Work with the Community Health Nurse, and other community health staff to coordinate health education activities for diabetes education and prevention.
  • Prepare modified diet plans for clients with specific health problems.
  • Provide instruction to clients in the use of modified diets.
  • Attends outside meetings as requested by the Director.
  • Attend continuing education workshops as requested by Director or as required to maintain certification.
  • Maintain a library of accurate and appropriate education materials as an adjunct to counseling and education.
  • Provide an initial nutritional assessment for each client.
  • Be responsible for following confidentiality procedures as outlined in the Privacy Act
  • Receive referrals from physicians, counties, and hospitals. Develop care plan and discharge planning for each new referral.
  • Develop program for early detection, prevention, and education of diabetes mellitus.
  • Perform nutritional assessments for Early Head Start (CWOO).
  • Attends quarterly Tribal Health Director’s meetings with the Minnesota Department of Health.
  • Schedule monthly meetings regarding health education, nutrition, and prevention.
  • Maintain diabetes registry for all community members.
  • Work with CHN and CHR to manage patients when necessary.
  • Assist in office duties when necessary.
  • Comply with all rules in the employee policies.
  • Complete HIPAA Training.
  • Regular attendance and punctuality necessary.
  • Available and willing to work any changes in hours on the schedule at any time during employment.
  • Other duties as assigned.

SKILLS KNOWLEDGE AND ATTRIBUTES :

  • Excellent verbal and written communication skills.
  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
  • Excellent organizational skills and attention to detail.
  • Proficient with Microsoft Office or related software.
  • Sensitive to Native American Culture.
  • Ability to maintain strict standards of confidentiality.

PHYSICAL REQUIREMENTS :

While performing the duties of this job, the employee is regularly required to talk and hear clearly, both in person and on the telephone.

The employee frequently is required to sit for long periods of time, stand and walk around offices. The employee must frequently lift and / or move up to 10 pounds and occasionally lift and / or move up to 25 pounds.

Work settings include office and entering community homes. Travel and exposure to typical weather seasons is necessary.
