Title : SCHOOL OUTREACH & FAMILY SUPPORT COORDINATOR
Reports To : Assistant Director School Outreach & Family Support
Location : 2471 Morris Ave.
Bronx, NY 10468
Hours : 35 hours, Non Exempt
Salary Range : 48-53K based on relevant experiene and education
Job Summary : The School-Based Early Support (SBES) Program is a new program providing counseling and case management services to families connected with PS / MS15, PS291, PS 246 or live within those surrounding communities.
The School Outreach & Family Support Coordinator is responsible for providing case management services to families referred from the Assistant Director School Outreach & Family Support.
The School Outreach & Family Support Coordinator supports the Assistant Director with school partnership activities and facilitating Triple P groups.
Major Duties :
Conduct assessments of children and families referred by the Assistant Director.
Provide case management services based on assessments using a strength-based approach.
All children of assigned families must be assessed each month.
Maintain participant case records in accordance with the program’s policies and procedures.
Conduct home visits and escort program participants to appointments as needed.
Partner with the Assistant Director to plan, coordinate and attend at each school social activities for children, parents, and families.
Prepare appropriate statistical reports.
Attend and participate in program staff meetings.
Participate in school meetings when indicated.
Facilitate with the Assistant Director, the Triple P group for all families involved in the school.
Implement and integrate the principles and procedures of Solution Based Casework : assessment, family and individual level outcomes, action plans and documentation and celebration.
Actively participate in case consultation, continued SBC training and the SBC certification process.
Perform other duties as assigned.

Qualifications :
BSW / BA preferred; will consider HS diploma with related experience.
Bilingual (English / Spanish) required.
Must be available to work early mornings and evening hours to accommodate parent and school personnel needs.
Willing to travel to each school and be on site on a regular basis.
Excellent organizational, written, and communication skills.
Must be proficient in Microsoft Office.
Must have a commitment to work from a strength-based, trauma-informed, anti-racist, youth and family development perspective.
