Description of the role :
We are hiring an LPN Care Coordinator to join our team at HR Employer Solutions Group.
As an LPN Care Coordinator, you will be responsible for coordinating and overseeing care services for our clients.
You will play a vital role in ensuring the well-being and satisfaction of our clients by developing care plans, coordinating with healthcare professionals, and providing excellent customer service.

Responsibilities :
- Develop and implement comprehensive care plans for clients
- Coordinate and schedule services with healthcare professionals
- Monitor and evaluate the effectiveness of care plans
- Communicate with clients and their families to ensure their needs are met
- Collaborate with internal teams to identify and address any issues or concerns
- Maintain accurate and up-to-date client records
- Provide excellent customer service and support to clients
Requirements :
- Valid LPN license
- Prior experience in care coordination preferred
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Compassionate and empathetic nature
