Job Summary
We are looking for a Home Care Aide to join our team at our current locations in Mesa, Chandler, Tempe, Gilbert, Phoenix, Glendale and Queen Creek!!!!!
You will be directly working with patients, following a one-on-one care Person Centered Plan in an up to 3 patient home. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve.
You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role.
Prior Home Health or Hospice experience is a plus, but not necessary Agency will train. Paid Agency Training and assistance with State required trainings and licenses.

Responsibilities
- Assist patients with daily activities such as hygiene care, appointments completions, Outings into community, Mealtime prep / assistance
- Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
- Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor
- Build relationships with patients by interacting, conversing or reading aloud to help keep them mentally healthy and alert
- Give and document medications
- Engage patients in exercises, dance, movies, board games, video gaming, or other activities
- Assist Behavioral Specialist with hands off behavior techniques (Hands on only by certified staff in severe 911 cases only)
Qualifications
- CPR certification required, can obtain when hired
- First Aid required, can obtain when hired
- Valid Fingerprint Clearance card required, can obtain when hired
- Article 9 (Rights of person with disabilities) required, can obtain when hired
- Prevention and Support (certified support training) required, can obtain when hired
- Abuse and Neglect required, can obtain when hired
- Clean and Valid AZ Driver's License and ability to transport in company's vehicle extra pay
- Documented Health Care or Behavioral Management Experience extra pay
