Description
Under the supervision of a collaborative physician, the Nurse Practitioner Resident assesses, diagnoses, and treats certain chronic health conditions in specific populations.
Training is provided relevant to the population served, based on scope of care of the service assignment. The Nurse Practitioner Resident acts as the primary contact and point of continuing care for patients within a community-based healthcare system while coordinating other specialist care that the patient may need.
Position holder denotes 100% effort for 12 months. This grant position is funded by the Health Resources and Services Administration's (HRSA) program.
The position is annually funded through the grant end date only.
Job Duties
- Obtains complete or interval patient history
- Performs assessments via observation, interview and examination, as indicated.
- Orders, performs, and interprets diagnostic studies.
- Obtains appropriate laboratory specimens.
- Identifies patient risk factors.
- Performs preventative health assessments, screenings, immunizations and care.
- Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
- Provides counseling and education of patients and their families / caregivers concerning preventative health, treatment options and community resources.
- Provides care in the context of promoting age and culture-appropriate informed choices, shared decision- making and the right to self-determination.
- Provides accurate and timely documentation of assessment and patient care plan.
- Assures treatment and / or plan of care are consistent with established protocols as well as patient preferences.
- Assures appropriate consent forms are signed and dated.
- Refers patients to appropriate services that exceed those of the clinic or scope of practice of the practitioner.
- Assists in the maintenance of adequate patient health records.
- Completes rotations with specialty populations in healthcare, most likely in rural and underserved areas.
- Participates in an interprofessional team-based clinical practice training model.
- Adheres to all organizational policies and procedures.
- Follows established medical and specialty protocols.
- Executes all tasks and behaves in a manner consistent with a culture of compliance, safety, and high reliability.
- Presents behavior supporting the organization's mission, goals, and core commitments.
- Travels to Samford University or other designated sites to complete continuing education or training.
- Participates in scheduled didactic and experiential learning activities.
- Completes online training modules.
- Participates full-time in the Nurse Practitioner Residency Program.
Qualifications
- Graduate from an accredited school of nursing and nurse practitioner primary care program between April 1, 2023 and August 30, 2024.
- Graduate with a Master's degree or higher as a Family Nurse Practitioner or Adult Gerontology Primary Care Nurse Practitioner prior to August 30, 2024.
- Unencumbered license as a registered nurse in the state of Alabama.
- Citizen of the United States.
- National certification from the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP).
- Occasional lifting / carrying objects up to 50 pounds.
- Ability to work daily, five days per week, including weekends.
- Ability to work continuously throughout the day.
- Ability to frequently bend / stoop, reach, stand, walk, and utilize repetitive hand / arm movements.
- Ability to frequently lift / move patients.
- Ability to keyboard / data entry for extended periods of time.
- Ability to work with data of a confidential nature.
- Excellent written and verbal communication skills.
- Positive and effective interpersonal skills to respond to a variety of constituents.
- Effectively manage multiple tasks in an organized manner.
- Must be a citizen of the United States, a non-citizen national, or a foreign national who possesses a visa permitting permanent residence in the United States.
Preferred Qualifications
Prior experience with electronic health records.
