Position Summary :
The Home Care / Hospice registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.
The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and / or assisted care in their community setting.
The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes : admission, discharge, billing, referrals, bereavement, and care delivery.
Work Schedule : M-F, every 5th weekend
Required Qualifications :
- Education, Experience or Equivalent Combination : Work experience as a RN for at least two years
- Licensure / Registration / Certification : Current MN Registered Nursing License
- Knowledge, Skills, and Abilities :
- Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
- Proven proficiency in the operation of the following : Personal Computer, Telephone, Fax Machine, Photocopier, Printer, and electronic medical record.
Preferred Qualifications :
- Experience in home care and or hospice desirable.
- Must be able to function independently with minimal supervision.
- High level of sensitivity, assessment skills, technical-skills and communications skills.
Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
