Job Summary :
The MDS Nurse plays a crucial role in coordinating and overseeing the Minimum Data Set (MDS) process within the SNF facility.
This position requires a deep understanding of nursing care, regulatory compliance, and effective communication skills. The primary goal is to ensure accurate resident assessments, adherence to regulatory requirements, and improved overall quality of care.
MDS Coordination :
- Oversee the entire MDS process, from initial assessment to ongoing updates, ensuring accuracy and compliance with regulatory standards.
- Collaborate with interdisciplinary teams, including nurses, therapists, and other healthcare professionals, to gather and evaluate resident data for MDS completion.
- Stay abreast of federal and state regulations related to long-term care facilities and MDS requirements.
- Implement changes in regulations and communicate updates to the nursing home staff.
- Analyze MDS data to identify trends and areas for improvement in resident care.
- Collaborate with the Quality Assurance team to develop and implement strategies for enhancing the quality of care based on MDS assessments.
- Foster effective communication and collaboration among different departments to enhance the overall quality of resident care.
- Participate in care planning meetings and provide insights based on MDS assessments.
- Generate reports on MDS data, trends, and outcomes for leadership and regulatory agencies.
Qualifications : -
- Must have experience working in an SNF as an MDS Care Plan Nurse
- Registered Nurse (RN or LPN) with a valid Illinois nursing license.
- Must be updated on current CMS Changes and regulations
- Good communication skills and ability to train MDS staff on new regulations
