As part of the Multi-disciplinary Treatment Team, provides direct clinical social work services to clients in the abuse intervention program targeting behaviors that led to domestic violence.
Responsibilities include clinical assessment, clinical consultation, therapy, discharge planning, post-discharge follow-ups, and client-centered advocacy.
- Interview the client, their families, and other persons / agencies involved to obtain significant social and other data needed for diagnosis and treatment planning.
- Conduct assessments to establish a plan to enhance the client's capacity for social functioning and community reintegration.
- Facilitates FVPP curriculum as well as individual sessions.
- Maintain an extensive directory of resources relative to health care, social services, housing, social entitlement and disability agencies, and vocational assistance programs.
- Identify and implement appropriate referrals and linkages in the maintained extensive directory or resources.
- Assists in coordinating the Multi-Disciplinary Treatment Team for all participants to collaborate and coordinate client treatment according to risk assessment, treatment, behavioral monitoring, and management of clients.
- Prepares reports containing findings, conclusions, and recommendations as required by funding source.
- Document client employment and progress in the client's case file, and collect data as required by funding sources.
- Interact verbally and nonverbally with clients, staff, and outside contacts professionally with respect and courtesy.
- Maintain client and staff confidentiality in compliance with administrative policies and procedures.
- Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
- Other duties assigned by Supervisor.
