Qualifications :
- Must have a PharmD degree.
- A minimum of two years of experience in a hospital or similar healthcare system.
Hours and Compensation :
- This is a full-time position.
- Salary range is between $118K-$162K, depending on experience.
- Comprehensive benefits package included. Relocation assistance available if needed.
Responsibilities :
- Provide general clinical pharmacy services, including . development and reporting, and educational opportunities.
- Compound and dispense medications to patients, offering drug information and education to optimize care quality.
- Orient new personnel and students to department services and programs.
- Conduct educational in-service programs to share the latest treatments and therapies with other care providers.
- Communicate with prescribers to clarify medication orders and address unobtainable medications, reducing errors and therapy delays.
- Assist with inventory management and ordering pharmaceuticals to minimize stock outages and maintain appropriate levels.
- Support procurement of quality generic products and formulary practices to provide high-quality care at an affordable price.
- Maintain medication on assigned patient care areas and ensure emergency carts are well-prepared for immediate use.
- Calculate caloric and electrolyte ingredients for TPN to ensure accurate administration of patients' nutritional therapy.
- Provide timely and accurate drug and toxicological information to customers to maximize knowledge and minimize risk.
- Accurately enter medication orders into the computer system and monitor drug therapy for potential problems.
- Personalize drug dosing for patients based on pharmacokinetic / pharmacodynamic principles.
- Respond to and support Code Blue and Code Trauma situations to enhance patient survival and stabilization.
- Adjust medication doses for patients with renal or hepatic function impairment to avoid potential toxicities.
- Individualize medication doses for neonatal, pediatric, and geriatric patients.
- Calculate and compound chemotherapeutic agents for cancer patients.
- Make recommendations to management for formulary positions and approaches that reduce costs while maintaining / improving pharmaceutical care.
- Familiarity with spreadsheet and clinical-based computer programs to enhance data manipulation and competitiveness in a managed care environment.
- Assist management in the administration of approved investigational drug research protocols.
