SUMMARY
Under the direct supervision of the Program Supervisor, the Case Manager performs general functions including providing case management to pregnant and parenting women and infants.
Community-driven strategies are utilized to address factors contributed to infant mortality, low birth weight and adverse perinatal outcomes in high-risk populations.
Family Case Management focus is to improve maternal and child health outcomes by increasing access to health services. Case Managers assist with coordinating care, accessing resources, providing on-going health education, removing and / reducing barriers.
The Case Manager provides excellent quality care, positive customer service, the maintenance of an efficient and safe environment of care.
Demonstrates superior customer service skills and has the ability to function in a multi-tasking, multiple priorities environment, while maintaining accuracy and attention to detail.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Maintain patient confidentiality at all times per the policies and procedures.
- Maintain ethical behavior at all time per the policies and procedures.
- Responds to internal and external customer needs in a positive manner, utilizing principles of the AIDET model; exceeding their needs and expectations and providing the highest quality service to ensure the best possible outcomes.
- Ability to utilize and appropriately document and retrieve data and information from Athena and / or other required Case Management / WIC portal (Cornerstone) per program requirements.
- Determine program eligibility by conducting risk screenings on pregnant and parenting women and program enrollment, if eligible.
- Conduct initial and periodic assessments of needed health and social service needs.
- Coordinate individualized care plan and oversee the completion of all recommendations suggested in the care plan including referrals and resources to clients.
- Assist participants with goal attainment by providing continuous follow-up.
- Assist participants with accessing medical and support services.
- Conduct developmental screening on infant and children participating in programs.
- Conduct mental health screening on women participating in programs.
- Provide face to face visits and home visits according to risk level.
- Meet and submit weekly program productivity numbers.
- Work as part of a multi-disciplinary team in coordinating and providing services.
- Attend all trainings and meetings as required by the funder.
- Complete all Core Curriculum training(s) required for this position.
- Perform all other duties as assigned.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and / or ability required.
EDUCATION and / or EXPERIENCE
- A.A. Degree in Human Service Field with 2 years of experience working with women and children or B.A. Degree in Human Services or related field
- Experience in working with Microsoft Office programs essential; adept at data entry
OTHER QUALIFICATIONS
Current driver’s license
Current auto insurance
Ability to work late evening hours, weekend hours, and scheduled holidays, as needed
