PURPOSE :

The Team Leader's main functions are to assist persons served to identify, review, and clarify their expectations / personal outcomes, design a person centered service plan, coordinate the activities of support teams, monitor / evaluate effectiveness of plans and advocate for changes based on personal outcomes.

Specific responsibilities follow :

ESSENTIAL JOB FUNCTIONS :

Leadership

  • Act as an advocate to assist people who have developmental disabilities live fulfilling lives as citizens.
  • Maintain professional interpersonal / communication skills to act in a facilitative role in all service settings on behalf of the person served.
  • Maintain ability to work within a support role to the person served and address issues / assist other support team members in doing the same.
  • Locate, arrange, monitor and evaluate community resources and program supports / services provided with regard to the identified outcomes of the person served.
  • Mediate and assist in resolution of disputes between service providers and person served.
  • Stimulate and assist in the development of new supports and services or modification of existing supports and services to better meet outcomes of person served.
  • Ensure the protection and promotion of rights of persons served to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights and the right to pursue complaints.

Assist persons served in the self-advocacy of their rights.

Person Centered Planning :

  • Facilitate / coordinate the development of a plan which focuses on personal outcomes yet supports vitality and imagination within.
  • Communicate / train plan components and monitor / participate in the implementation of support / services described within the plan.
  • Regularly assess the plan with regard to ever-changing needs, preferences and outcomes of person served and modify the focus of service plan as needed.
  • Provide for and / or follow up with any necessary supports / services (consultations, counseling, evaluations, transportation, physician appointments and staff training) identified.
  • Ensure that all personnel across service settings are appropriately trained to implement and maintain integrity of individual service plan delivery.

Communication / Documentation :

  • Complete all necessary documentation with regard to coordination of supports and services in a timely manner (ISP, QMMRs, Progress Notes, etc.).
  • Approve and ensure the processing of all service recommendations and referrals for services. Maintain documentation of such.
  • Responsible for all data collection systems and monitoring tools to be used in for the review / modifications of individual service plan / programs.
  • Interpret and analyze all data regarding program progress and appropriateness, making revision as necessary.
  • Receive and disseminate all information regarding ISP to appropriate parties / support team members.
  • Maintain regular communication with all support team members to ensure cohesive delivery of services.

DUTIES AND RESPONSIBILITIES :

  • Assist in maintaining compliance with current accreditation standards, all state and federal laws / regulations and agency policy.
  • Strive to improve knowledge and abilities in order to be most effective and efficient in job responsibilities that lead to the fulfillment of agency philosophy, values and mission statement.
  • Work a flexible schedule adapted to the individualized needs of person served. Complete all necessary paperwork to accomplish the above listed tasks and oversee the efficient operation of Support Teams.
  • Serve on agency committees as assigned.

Qualifications :

A Qualified Intellectual Disabilities Professional (QIDP) must have at least one year of experience working directly with people with developmental disabilities, meet the minimum federal educational requirements for a QIDP.

Direct experience may be obtained on the job.

Direct work experience may include, but is not limited to activities that include involvement with consumers in daily, work or social activities;

interactions with consumers in the implementation of individual service plans, education plans or behavior programs, or involvement with consumers in programming at residential, work sites and other venues.

Educational credentials includes being a Human Services professional holding a bachelor's degree in human services field, including but not limited to sociology, special education, rehabilitation counseling and psychology, Registered Nurse, Occupational Therapy Assistant, Physical Therapy Assistant, Professional Recreation Staff Person, and / or Dietician.
