JOB TITLE : Medical Records and Referral Coordinator

DEPARTMENT : Medical Clinic

SUPERVISOR : Director of Clinical Operations

SUMMARY

Gather, process and maintain medical records in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health system.

Provide follow-through on patient referrals made in and out of PCCS departments including scheduling and tracking referral appointments and correspondence.

RESPONSIBILITIES

• Maintain patient confidentiality in accordance with HIPAA and clinic procedures.
• Release information to individuals and agencies according to regulations and in a timely manner.
• Manage and review all incoming RIGHT FAXES and documents to ensure they meet compliance requirements.
• Recognize and report chart concerns and errors.
• Review records for completeness, accuracy, and compliance with regulations before scanning documents.
• Scan and upload documents and results into EHR (electronic health record).
• Data entry of test results and send to provider.
• Respond to all inbound and outbound documentation requests.
• Receive and manage all referrals made in Epic
• Schedule appointments for patients with a referral order
• Track all referrals and complete a phone follow-up encounter as necessary
• Assist with other duties as assigned

KNOWLEDGE, SKILL AND ABILITY

• High School Diploma or equivalent.
• An associates or bachelor’s degree in a healthcare-related field or at least 1 year of experience in a similar role preferred.
• Knowledge of medical terminology and clinical results
• Experience with OCHIN / Epic or electronic health records
• Experience with and knowledge of computers, computer software, and RIGHT FAX .
• Excellent customer service skills with an ability to establish and maintain working relationships with patients, employees and the public.
• Ability to communicate effectively verbally and in writing
• Ability to work with people of diverse backgrounds and experiences.
