JOB TITLE : Medical Records and Referral Coordinator
DEPARTMENT : Medical Clinic
SUPERVISOR : Director of Clinical Operations
SUMMARY
Gather, process and maintain medical records in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health system.
Provide follow-through on patient referrals made in and out of PCCS departments including scheduling and tracking referral appointments and correspondence.
RESPONSIBILITIES
- Maintain patient confidentiality in accordance with HIPAA and clinic procedures.
- Release information to individuals and agencies according to regulations and in a timely manner.
- Manage and review all incoming RIGHT FAXES and documents to ensure they meet compliance requirements.
- Recognize and report chart concerns and errors.
- Review records for completeness, accuracy, and compliance with regulations before scanning documents.
- Scan and upload documents and results into EHR (electronic health record).
- Data entry of test results and send to provider.
- Respond to all inbound and outbound documentation requests.
- Receive and manage all referrals made in Epic
- Schedule appointments for patients with a referral order
- Track all referrals and complete a phone follow-up encounter as necessary
- Assist with other duties as assigned
KNOWLEDGE, SKILL AND ABILITY
- High School Diploma or equivalent.
- An associates or bachelor’s degree in a healthcare-related field or at least 1 year of experience in a similar role preferred.
- Knowledge of medical terminology and clinical results
- Experience with OCHIN / Epic or electronic health records
- Experience with and knowledge of computers, computer software, and RIGHT FAX .
- Excellent customer service skills with an ability to establish and maintain working relationships with patients, employees and the public.
- Ability to communicate effectively verbally and in writing
- Ability to work with people of diverse backgrounds and experiences.
