This position provides direct, hands-on assistance to an individual in their home to enable the individual to live and thrive in the community.
Duties depend on the individual’s needs and capabilities, and generally include assistance with activities of daily living, socialization, and behavior modification.
Assistance to individuals is provided in conformance with the ISP, Behavior Plan, and other team-established and individual-expressed needs and goals for that individual.
