Educator - Diabetes

The Educator uses the educational design process to close the knowledge, skills, and / or practice gaps identified through needs assessment and to impact evidence-based practice.
Essential Duties and Responsibilities :
Contributes to the professional development of others and supports life-long learning as individuals transition across learning / practice environments, and professional roles.
Applies a systematic process to shift individuals, teams, and organizations from current state to desired state and close performance gaps.
Applies a variety of learning technologies to address learning and performance needs, encourage knowledge sharing, and promote collaboration.
Provides leadership to project and learning activity implementation, scientifically measuring the impact on organizational goals.
Helps establish and participates in an education program teaching self-control skills to persons with diabetes and their families / significant others.
This education program includes an assessment, individualized implementation plan and follow-up evaluation.
Obtain regular continuing education in areas of diabetes management and will also participate in The Endocrine Group's continuous quality improvement process.
Qualifications :
- Communication
- Presents self in a professional courteous manner.
- Establishes and maintains professional rapport with the physicians and office staff.
- Establishes and maintains good rapport with patients and significant other appropriately.
- Customer Relations
- Identifies self with name when answering the telephone.
- Greets each patient by introducing self.
- Responds appropriately to patient requests as they occur.
- Reports all patient complaints to the Diabetes Education Coordinator as the occur.
- Maintains strict patient confidentiality, with the exception of reporting all medical related information to the Diabetes Education Coordinator or Physician.
- Infectious Disease
- Follows appropriate use of Universal Precautions without deviation.
- Reports incident of observed or suspected non-compliance with Universal Precautions immediately to the Manager.
- Quality Improvement
- Participates in Continuous Quality Improvement program under the direction of the Coordinator.
- Welcomes new ideas, changes and responsibilities to achieve greater success and productivity within the organization,
- CDCES - Certified Diabetes Care and Education Specialist - preferred
- Didactic and experiential preparation in diabetes management - required
- RN - Registered Nurse - State Licensure and / or Compact State Licensure NYS - preferred
- RD - Registered Dietician - required
