RN CASE MANAGER

We are seeking a dedicated RegisteredNurse Case Manager to join our healthcare team. The ideal candidatewill be responsible for coordinating patient care providing medicalsupport and ensuring optimal patient outcomes.
Responsibilities
Maintain medicaldocumentation / charting on all assigned Service Members to includetracking medical. status and documentation on Service Membercontact (email voice call or personal interview) in eCase inaccordance with (IAW) AR 4066 and unit Standard OperatingProcedures (SOP).
Provide reports anddocumentation related to Service Member medical readiness factorsto DSS and unit command teams as requested and required.
Provide care coordination on all assigned Service Membersand maintain automated referrals per SOP and submit case processingmedical documentation and profile concerns to military / contractedprovider.
Coordinate Service Member treatmentplans with appropriate medical facilities to include requestingspecific exams and evaluations as needed.
AttendPHA events as needed.
Ensure Service Membersreferred by a provider are followed by the case management team.Manage Service Members care from the identification of thecondition to when the Service Member is either considered medicallyready or administratively separated including :
Obtain release of information (ROI) from ServiceMember.
Track medical appointments bycommunication with unit to ensure Service Member attendance andsubmission of follow up documentation.
Reviewand track health assessments (PHA SRP) lab results for accuracy andcompleteness.
Educate Service Members onresources available to improve medical and dental readinessincluding TRICARE benefits and possible Veterans Affairseligibility.
Service Members not turning in medical documents ornot seeking medical care for a reported issue due to lack ofinsurance is an unacceptable excuse and shall be reported asnoncompliance.
Communicate with delegatedgovernment personnel to track compliance versusnoncompliance.
Adhere to all laws regulationsand policies applicable to Individual Medical Readiness (IMR) anddeployment to include maintaining Health Insurance Portability andAccountability Act of 1996 (HIPAA) standards andguidelines.
Coordinate with Medical ReadinessNonCommissioned Officers (MRNCOs) military medical providers ortheir alternate to provide information to Service Membersleadership concerning medical updates and counseling.
Specificdetails shall be documented in eCase for the MRNCO toreview.
Inform military medical provider of theService Members status and furnish them with the mandatoryinformation / documentation as received and reviewed.
Build initialshell of temporary profiles based off civilian providerdocumentation within the EPROFILE database within 5 business daysof receiving civilian documentation.
Review alldocumentation and compose a narrative summary (NARSUM) of theinformation NLT 10 days following profile determination.
Write theNARSUM in memorandum format IAW AR 2550; and write the sameinformation using a medical note in the Service Members electronicchronologic progress note in MEDCHART eCase module.
Review clinical treatment plans and document treatmentplans as given from a Service Members treating Provider. The NurseCase Manager shall be responsible for transposing treatment plansfrom clinical documentation into ADOP packets INCAP packetsConvalescent leave packets and Medical Board packets whereapplicable.
If a Service Members clinical documentation is notspecific about the proposed treatment plan the shall call theService Members treating Providers Office and speak with thetreating Provider and / or nursing staff in order to obtain anappropriate treatment plan.
Document all casemanagement notes in MEDCHARTS eCase which all cases shall beassigned in.
Scan and digitally update allmedical documentation received by the Case Management team toMEDCHARTs Health Readiness Reporting (HRR) module IAW AR 4066 andState Surgeons Office guidance on indexing to various digitalfolders / repositories in HRR.
The Administrative Assistant willperform the majority of the case file indexing andupload.
Add all medical documentation to theSoldier Treatment Record (STR) in paper form IAW AR 4066. Followproper PII and HIPAA laws for medical organization and management.
When a Service Member is discharged from the IAARNG the ServiceMembers digital medical record file shall be validated IAWregulatory guidance.
Organize medicaldocumentation IAW regulatory guidance for disability evaluationsystem medical board packet specifics. Each medical board packethas a different requirement for medical document organization.
Nurse Case Managers and Administrative Assistants shall comply withguidance and procedure for composition of the individual medicalboard packets.
