POSITION TITLE : Care Coordinator Homeless Outreach and Community Health

CLASSIFICATION : Medical, regular, full-time, non-exempt

SUPERVISOR : Wellness Programs Director

POSITION OBJECTIVE

This position focuses on delivering appropriate, timely, and beneficial patient care, promoting quality and cost-effective healthcare outcomes while adhering to established standards for case management practice.

The role specializes in outreach services for the homeless and general services for all community health patients within The NATIVE Project, encompassing Family Medicine, Dental, Pharmacy, Behavioral Health, and Substance Use Treatment.

ESSENTIAL DUTIES

  • Establishes, develops, and maintains culturally appropriate interactions and connections with patients, staff, and the community.
  • Demonstrates the maturity and ability to function effectively in a fast-paced environment with a diverse patient population.
  • Works collaboratively within a team environment, utilizing medical office competencies and understanding written and verbal procedures and instructions.
  • Supports medical and behavioral health teams by managing consults, referrals, case management, patient follow-ups, and additional projects as required.
  • Provides resources and referrals within the organization and to other community partners.
  • Become a certified Patient Navigator, assisting patients in finding insurance through Washington Health.
  • Delivers outstanding customer service by greeting patients and staff politely, promptly, and helpfully, offering appropriate instructions or directions.
  • Proactively collaborates with all interdisciplinary team members.
  • Coordinates services aligned with the Patient-Centered Medical Home model.
  • Helps maximize patient healthcare outcomes.
  • Utilizes best practice models to identify, incorporate, or develop best practices for panel management.
  • Tracks patient follow-up and / or referrals for reporting purposes.
  • Maintains strict confidentiality and adheres to all HIPAA guidelines / regulations and all policies and procedures of The NATIVE Project.
  • Maintains a working knowledge of all programs and outreach offered by The NATIVE Project.
  • Attends regular staff meetings.
  • Performs other job-related duties as assigned.

SUPERVISOR RESPONSIBILITY

This position has no supervisory responsibilities.

WORK ENVIRONMENT

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

PHYSICAL DEMANDS

The employee is occasionally required to sit, climb, balance, stoop, kneel, crouch, or crawl. The employee must frequently lift and / or move up to 10 pounds and occasionally lift and / or move up to 25 pounds.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

REQUIREMENTS, EDUCATION & EXPERIENCE

  • Associate’s degree or higher in public health or social services preferred.
  • Experience working in Behavioral Health, Substance Use Treatment, and / or Community Health settings.
  • Two years of office experience required.
  • Demonstrated strong organizational skills and ability to manage multiple tasks and conflicting deadlines.
  • Effective oral and written communication skills.
  • Ability to work in a team environment in leadership, peer relationships, and conflict resolution.
  • Strong proofreading and editing skills with excellent grammatical skills.
  • Proficiency in operating various office equipment, such as a computer, photocopier, scanner, printer, postage meter, etc.
  • Proficiency in Microsoft Word or other word processing programs, with the ability to be trained in other software programs, such as Excel, PowerPoint, Outlook, Access, Greenway / Intergy, and Provider One.
  • Ability to pass a criminal background check, OIG check, and pre-employment UA / BAs.
