WHAT WE ARE LOOKING FOR :
Masters-prepared Social Workers to work with our renal patients and families to assist with the adjustment to what is likely a life-changing event.
Provide emotional and social adjustment counseling, assistance with removing barriers to treatment, education on kidney disease and treatment goals, and bridging barriers to referral services and community resources.
- Conducts initial psychosocial interviews with patients and families in order to assess baseline level of functioning, comprehension of illness and its ramifications, and impact of individual / family history upon current and future psychosocial functioning.
- Provides ego-supportive counseling to patients and their families in order to maximize ongoing psychosocial functioning.
- Provides crisis intervention to patients and families as needed.
- Functions as a patient advocate and as a liaison between the patient and multiple care delivery systems.
- Participates in comprehensive inter-disciplinary team patient care meetings with appropriate documentation on IDT assessment and plan of care.
- Records psychosocial assessments, update assessments and progress notes in accordance with established recording guidelines.
Education / Experience
- Masters of Social Work Degree required.
- Experience in a healthcare setting preferred.
- Demonstrates understanding of the impact of Chronic Kidney Disease (ESKD) on activities of daily living, quality of life and mental health needs.
- Familiar with both clinical and case management techniques.
