Intake Coordinator
Helping U Home Care (HUHC) is looking for an Intake Coordinator to join our exceptional interdisciplinary team of home health care providers.
Do you have experience working in the Home Care Industry with referral sources helping process their HHA / PCA, CDPAP, PDN or NHTD cases?
We are looking for compassionate, dedicated and well-rounded professionals who can uphold our standards and culture.
Qualifications
- Minimum 1 year of Intake experience working at a Licensed Home Care Agency
- Understanding the importance of timely follow-ups with referral sources, clients & patients
- Well-versed in NYS Medicaid, MLTC and CDPAP eligibility
- Ability to multi-task and communicate effectively, both verbally and in writing
- Experienced with Microsoft Office 360 and HHA Exchange
- Familiar with PDN (Private Duty Nursing) or NHTD / TBI a plus
Responsibilities
- Receives new HHA / PCA or CDPAP referrals and initiates the intake process
- Ensures required demographic & clinical information for each referral is obtained in a timely fashion
- Obtains authorizations for service requests
- Enters new referrals into HUHC's Intake System & notifies Coordination, Sales, and Clinical Departments
- Maintains constant communication with referral sources & patients / families until the referral is transferred to Coordination

- $45,000 - $50,000 per year
