Since 1984, Larkin Street Youth Services has helped more than 75,000 young adults in San Francisco with outreach, shelter, housing, education, employment, healthcare, and other services.

Today, Larkin Street Youth Services is the largest provider of housing and services to homeless youth in the Bay Area. We serve 1,500 youth annually, operating or partnering to provide more than 500 emergency shelter, transitional, supportive, and subsidy-based housing beds at several unique sites across San Francisco.

WHAT YOU'LL DO :

Under the supervision of the Program Manager, the Housing Navigator is to provide comprehensive strength-based case management services to homeless and / or marginally housed transitional-aged youth who are connected to TAY Housing Navigation.

The Case Manager II will provide support to youth accessing Emergency Housing Vouchers by providing a comprehensive evaluation of the bio-psychosocial, emotional, educational, vocational, and functional needs of young people seeking housing services and assistance.

The Case Manager II will utilize a trauma-informed, strengths-based model to help TAY identify and utilize a wide range of community resources to resolve their episode of homelessness.

The Case Manager will also offer services and support to youth after they exit the residential housing continuum and transition toward self-sufficiency.

Aftercare youth receive counseling, information, and referrals, and are invited to participate in Larkin Street events. Larkin Street staff link youth 25 and older to adult services and resources in the community.

The Program aims to maintain connections with youth who exit housing for 1-2 years post-exit. Such ongoing contacts allows Larkin Street to collect data on youth's progress toward the organizational outcomes of housing, education, employment, and wellness.

This is a full-time position, eligible for full benefits.

Your responsibilities :

Emergency Housing Voucher

  • Conduct intakes and assessments.
  • Provides weekly case management to youth accessing Emergency Housing Vouchers (EHV).
  • Completes client safety and wellness checks for EHV clients that are placed in SROs or hotel rooms in San Francisco Bay Area.
  • Works closely with community behavioral health providers and internal personal service teams to ensure coordinated and effective services.
  • Provides an intensive array of services and supports, including housing and basic needs assistance, physical health care, benefits assistance, enrollment in Larkin Street Academy, and referral to integrated mental health services.
  • Maintains accurate notes of client interactions and timely documentation in client management systems including, but not limited to, Efforts to Outcome and the ONE system.
  • Adheres to laws regarding confidentiality and reporting requirements and maintains necessary compliance documentation.
  • Uses motivational interviewing, stages of change, harm reduction, and other approaches, to build rapport with TAY and help them resolve their homelessness.
  • Collaborates with fellow staff members and established community partners, including property managers and landlords.
  • Provides coverage for program / site staff and performs duties in the milieu which may include desk coverage, cooking, and cleaning.
  • Provides transportation and accompaniment to client appointments in the community.

Aftercare Transitions

  • Ensure follow-up contact with clients during transitions from a residential program.
  • Support case management staff to consistently and correctly collect Aftercare data in SharePoint.
  • Regular meetings with the Associate Director to spot and correct problems with data collection.
  • Coordinate with Research and Evaluation to update data collection tools and practices.
  • Using regular Housing Transitions and Stabilization data reports to ensure contact with and support of clients.
  • Participates in organizational meetings (all-staff, CQI, client review, etc.)
  • Ability to teach and model life skills.
  • Responsible for supporting program services within the milieu. For example, crisis intervention or helping to maintain the safety of the program.
  • Responsible for contributing towards the care and welfare of staff and to the clients we serve.
  • Other duties as assigned.

Working Location :

  • ECC - Drop-In Center
  • Location : The Engagement and Community Center (ECC), 134 Golden Gate Ave, San Francisco, CA
  • Hours : 9AM to 5PM OR 10AM to 6PM, depending on site needs.
  • Working Days : Monday through Friday

  • You hold a valid driver's license.

  • $1.50 for bilingual incentive pay. Must be fluent in Spanish and English.
